Request a Health Declaration Form
In order to operate a vehicle, you must be physically and mentally healthy. You can demonstrate your health with a so-called Certificate of Medical Fitness (VvG). To get this certificate from the CBR, you need to fill out a Health Declaration Form. The form contains questions about your physical and mental health. In some cases, you will have to be examined by a physician. This certificate is not necessary for a moped licence.
When do you need it
A Health Declaration is necessary in the following cases:
- When you apply for your first driving licence and for each new licence category you obtain (does not apply to moped licence: category AM).
- If you are 65 or older and your current driving licence expires on or after your 75th birthday.
- If you are 75 or older and want to renew your driving licence.
- When applying for or renewing a C or D licence.

Fill out your Health Declaration Form

You can complete your Health Declaration Form online through My CBR. Or purchase a hard-copy version at a CBR location. A paper version will take longer to process. Please consult the CBR website for a current overview of CBR locations.

You can also purchase a Health Declaration Form at most municipalities. Check whether this applies to your municipality as well.

Submit the Health Declaration Form to the CBR.

The CBR will assess whether you are medically fit or whether an additional examination is needed.

If you are medically fit, the CBR will inform you in writing. No need to notify us.
The registration of the Medical Fitness Certificate is valid for 1 year. This means that you have 1 year to file for your driving licence at your municipality.
